Your First Visit

After we quickly check you in when you walk through the door, we’ll promptly welcome you back into one of our operatories where you’ll meet one of our periodontists. In total, this appointment can take up to an hour, ensuring that we are fully aware of your needs and goals and that you understand what you’re responsible for paying moving forward.

New Patient Forms

Every patient who is coming to our office for the first time needs to complete our new patient forms. This paperwork gives us a more complete medical and dental history, insurance information, and more. That doesn’t mean that you need to arrive extra early and spend additional time in our waiting room. You can download and print these forms beforehand and bring them already completed. That way, we can process your paperwork and have you ready to start on time.
